I am surprised this is still open for comments, but more surprised that a simple truth is not explained, that the Savior is the final judge. We all receive ordinances and can accept or reject them, but as to their efficacy, that is decided in the final judgment. No one else judges for eternity, even those managing the temple records.
Re: “The Baptism of Ted Bundy”
If spiritual discernment was perfect, we would not gain faith, which is one of the basics of the Gospel. Most comments here are wrong in many details. I know because I was in the same branch as Bundy and close to those who baptized him. I have theories as to why Bundy was allowed by God to be baptized, but I'm not in the mood to start an argument. The same question could be asked about why God allows any evil. It boils down to the purpose of this life. We are to help each other through these problems. Satan wanted a world where evil was not allowed, where interviews were all perfect.
