According to the Utah Division of Occupational and Professional Licensing, Dr. Larry Glen Andrew of Provo completed probation and had his licenses to practice medicine and administer controlled substances fully reinstated.
Meanwhile, Salt Lake City doctor James Allen Miller voluntarily surrendered his licenses after being found to have engaged in unlawful conduct including sexual involvement with patients he was prescribing drugs to and not keeping adequate patient records. Miller may not reapply for licensure until five years after the completion of his probation.
