New Doctors Added to CW's Discipline Database

Three new doctors have been added to City Weekly’s discipline database, including two doctors reprimanded for alleged substance-abuse concerns.---

David Max Anderson, MD, a Sandy physician, will have his license to practice and to administer controlled substances suspended on Nov. 9, 2011, in connection to allegations of abusing a controlled substance in regard to his dentistry practice. Anderson, while neither denying nor admitting to the allegations, has agreed to the terms of discipline set out by the Division of Occupational and Professional Licensing.

While DOPL's discipline notice didn’t specify the substance abused, it was classified as a schedule II controlled substance, which under Utah law covers opiates ranging from opium to hydrocodone.

Mark Hawthorne Keep, MD, voluntarily surrendered his license for unprofessional conduct while impaired by drugs and/or alcohol. He also was alleged to have made inappropriate sexual comments to “females in the workplace.”

Another doctor, Robert Weitzel, his license to practice and administer controlled substances, was placed on probation, only noting discipline on his license in other states such as Texas and California.
